Stany przedrakowe skóry to choroby, na podstawie których może rozwinąć się nowotwór. Dzielą się one na bezwzględne (obligatoryjne) oraz względne (fakultatywne). Czym charakteryzują się te schorzenia?
Bezwzględne stany przedrakowe, in. przedinwazyjne nowotwory charakteryzują się tym, że nie przekraczają granicy skórno-naskórkowej. Zalicza się do nich rogowacenie słoneczne, skórę pergaminowatą i barwnikową, rogowacenie popromienne, arsenowe i smołowcowe, Chorobę Bowena, erytroplazję Queyrata, róg skórny oraz rogowacenie białe.
Zmianami skórnymi, które sprzyjają rozwojowi raka, jednak charakteryzują się niskim ryzykiem rozwoju są stany względne. Objawiają się w postaci owrzodzeń, blizn czy przetok.
Rogowacenie słoneczne jako stan przedrakowy skóry
Rogowacenia słoneczne to liczne zmiany, które pojawiają się najczęściej na czole, nieowłosionej głowie, małżowinach usznych oraz na skroniach. Niekiedy występują również na wierzchniej stronie rąk oraz na podudziach. Schorzenie to jest nawarstwieniem rogowym o zabarwieniu żółtobrunatnym. Zmiany te charakteryzują się suchą, nierówną powierzchnią. Zazwyczaj są tego samego poziomu co skóra wokół, jednak niekiedy mogą nieco wystawać.
W leczeniu rogowacenia słonecznego stosuje się zamrażanie płynnym azotem, 5% maść fluorouracylową, tretynoinę w kremie 0,05-0,1%. W przypadku wystąpienia objawów przejścia w raka in situ, niezbędne jest wówczas chirurgiczne usunięcie lub głębsze zamrażanie płynnym azotem.
Skóra pergaminowa i barwnikowa jako stan przedrakowy skóry
Do tego typu skóry zalicza się piegi, plamy soczewicowatej, odbarwienia, zaniki, teleangiektazje. Występują stosunkowo rzadko i zazwyczaj rodzinnie. Najbardziej zagrożona tym schorzeniem jest skóra odsłonięta.
Aby uniknąć tego typu zmian należy chronić skórę przed promieniami słonecznymi. Należy również pamiętać o usuwaniu zmian przednowotworowych i doustnym stosowaniu aromatycznych retinoidów.
Rogowacenie popromienne jako stan przedrakowy skóry
Do powstania tego rodzaju zmian dochodzi na podłożu skóry, na której wystąpiły liczne teleangiektazje, zmiany barwnikowe, nierównomierne stwardnienia, przebarwienia czy zaniki bliznowate.
Można się ich pozbyć poprzez chirurgiczne usunięcie zmian przedrakowych, bądź kriochirurgię. W przypadku wystąpienia licznych tego rodzaju zmian, stosuje się 5% maść 5-fluorouracylową.
Rogowacenie arsenowe
Zmiany tego rodzaju to rogowacenie dłoni i stóp, ogniska raka podstawnokomórkowego, a także rozsiana plamkowa hiperpigmentacja. W leczeniu rogowaceń arsenowych również stosuje się chirurgiczne usuwanie lub kriochirurgię. Natomiast w przypadku licznych zmian – 5% maść fluorouracylową.
Rogowacenie smołowcowe
Stan ten charakteryzuje się uczyleniem na światło, dotykającym głównie twarzy, szyi i ramion. Wówczas mamy do czynienia ze zmianami przypominającymi skórę pstrą. Po wielu latach z tych zmian tworzą się brodawczaki smołowcowe. Leczenie w tym przypadku wygląda tak samo, jak u pacjentów z rogowaceniem arsenowym czy popromiennym.
Choroba Bowena
Zmiany występujące przy tym schorzeniu wyglądem przypominają złuszczone plamy/ tarczki. Pojawiają się zazwyczaj na tułowiu, a niekiedy też na twarzy, narządach płciowych i palcach. W przypadku wystąpienia na błonach śluzowych, nazywane są wówczas erytroplazją Queyrata. Leczenie tej choroby również polega na chirurgicznym usunięciu zmian, krioterapii lub zastosowaniu 5% maści fluorouracylowej.
Erytroplazja Queyrata
Zmiany towarzyszące tej chorobie to czerwone, aksamitne plamy/ tarczki. Najczęściej występują na prąciu, jednak niekiedy również na żeńskich narządach płciowych, w okolicy odbytu czy jamie ustnej. W leczeniu tego schorzenia stosuje się krioterapię i laseroterapię.
Róg skórny
Twór rogowy charakteryzuje się żółtobrązową barwą. Rośnie pionowo do powierzchni skóry, zazwyczaj na skroniach, policzkach i małżowinach usznych. Najczęściej występuje pojedynczo. Tego typu zmiany usuwa się chirurgicznie.
Rogowacenie białe
Te białe, gładkie plamy niekiedy zaczynają przybierać postać brodawki. Występują na błonach śluzowych i półśluzówkach jamy ustnej i narządów płciowych. Na pojawienie się tego rodzaju zmian w jamie ustnej mają czynniki drażniące, takie jak palenie tytoniu, próchnica czy źle dopasowana proteza zębowa. Natomiast w obrębie narządów moczowo-płciowych wpływa marskość sromu czy żołędzi. Najważniejszym czynnikiem w leczeniu tego rodzaju zmian jest usunięcie czynników drażniących. U pacjentów z tego rodzaju zmianami stosuje się krioterapię, natomiast jeżeli zmiany są brodawkowate – ich chirurgiczne usunięcie.
