Długotrwała ekspozycja na niską temperaturę i wilgoć może doprowadzić do rozwoju stopy okopowej. Jej objawami są ból, maceracja skóry, obrzęk i martwica tkanek. Jeżeli wcześnie nie podejmie się leczenia, konieczna może być amputacja kończyny.

Stopa okopowa, nazywana też zespołem stopy zanurzeniowej, była powszechnym schorzeniem wśród żołnierzy walczących w okopach podczas I wojny światowej. Współcześnie występuje głównie u osób w kryzysie bezdomności. Na jej rozwój narażeni są także sportowcy wyczynowi, zwłaszcza uprawiający wspinaczkę wysokogórską, oraz osoby pracujące na zewnątrz w niesprzyjających warunkach atmosferycznych, takich jak niskie temperatury i wysoka wilgotność. Czynnikami zwiększającymi ryzyko rozwoju tego schorzenia są również otyłość i schorzenia powodujące zaburzenia przepływu krwi, np. cukrzyca.

Jak powstaje stopa okopowa?

Rozwój stopy okopowej jest konsekwencją współwystąpienia kilku czynników środowiskowych, m.in. długotrwałej ekspozycji stopy na wilgoć oraz temperaturę poniżej 10 st. C. Następstwem narażenia stopy na takie warunki są zmiany odmrożeniowe. Pod wpływem zimna naczynia krwionośne ulegają obkurczeniu. Niedotlenione tkanki nie otrzymują wystarczającej ilości składników odżywczych. Zaczynają odkładać się w nich szkodliwe produkty przemiany materii. Skóra staje się wówczas podatna na podrażnienia. Pod wpływem wilgoci naskórek ulega zmiękczeniu i puchnięciu. Tym samym staje się wrażliwy na uszkodzenia, zwłaszcza te wywołane tarciem.

Kolejnym czynnikiem, który powoduje rozwój stopy okopowej, jest przewlekły ucisk spowodowany np. zbyt ciasnym obuwiem. Ucisk skutkuje brakiem cyrkulacji powietrza, czego konsekwencją jest m.in. nadmierna potliwość stóp.

Wystąpienie stopy okopowej jest także związane z powtarzającym się przeciążeniem kończyn dolnych. W większości przypadków dochodzi do niego, gdy podczas poruszania się chory odciąża jedną część stopy, tym samym przeciążając inną. Przeciążenia mogą być też konsekwencją nadmiernego wysiłku fizycznego.

Jakie są objawy stopy okopowej?

Wczesne objawy

W pierwszym stadium choroby na palcach i podeszwach stóp pojawia się zaczerwienienie. Wkrótce dochodzi też do maceracji skóry. Uszkodzenia wierzchnich warstw naskórka mogą rozwinąć się w nadżerki, a następnie w rozległe owrzodzenia.

W dalszym przebiegu schorzenia pacjent zaczyna doświadczać drętwienia i samoistnych bólów nóg. Dolegliwości są szczególnie nasilone w obrębie palców. Ze względu na ból pacjent podczas chodzenia próbuje odciążyć palce. W konsekwencji przeciąża piętę, która staje się podatna na uszkodzenia. Na stopach występuje również obrzęk.

Zaawansowane objawy

Jeżeli niesprzyjające czynniki środowiskowe nie zostaną wyeliminowane, zmiany chorobowe postępują w głąb tkanek. Dochodzi do uszkodzeń włókien nerwowych, co powoduje stopniową utratę wrażliwości na dotyk i bodźce termiczne. Opuchlizna i zaczerwienienie obejmują już nie tylko stopy, ale także podudzia. Stopy stają się sine. Skóra wygląda na napęczniałą i napełnioną wodą. Pojawiają się ropne pęcherze, na których tworzą się strupy. Po odpadnięciu strupów pozostają ubytki. Może dojść do zgorzeli tkankowej i martwicy rozpływnej.

Powikłania stopy okopowej

Przewlekłe niedotlenienie tkanek stopy może doprowadzić do nieodwracalnych uszkodzeń nerwów i naczyń krwionośnych. Ropny wysięk z pęcherzy i przewlekły ucisk sprzyjają rozwojowi zakażeń bakteryjnych i grzybiczych w obrębie ran.

Na czym polega leczenie stopy okopowej?

Jeżeli w obrębie stóp nie powstały rany, powinno się przede wszystkim zadbać o właściwą pielęgnację skóry. W tym celu stopy należy utrzymywać w suchości i czystości. Zaleca się także stosowanie nawilżających maści i kremów, np. emolientów.

Terapia ran i owrzodzeń, do których doszło w przebiegu stopy okopowej, wymaga wdrożenia postępowania z raną przewlekłą zgodnego ze strategią TIME. Jeżeli zmiany martwicze są rozległe, konieczna może okazać się amputacja stopy.

It’s very colorful and lovely OLED screen is slightly smaller than the touch screen, but the player itself feels quite a bit smaller and lighter. It weighs about 2/3 as much, and is noticeably smaller in width and height, while being just a hair thicker.

The interface is colorful, has more flair, and some cool features like 'Mixview’ that let you quickly see related albums, songs, or other users related to what you’re listening to. Clicking on one of those will center on that item, and another set of „neighbors” will come into view, allowing you to navigate around exploring by similar artists, songs, or users. Speaking of users, the Zune „Social” is also great fun, letting you find others with shared tastes and becoming friends with them. You then can listen to a playlist created based on an amalgamation of what all your friends are listening to, which is also enjoyable. Those concerned with privacy will be relieved to know you can prevent the public from seeing your personal listening habits if you so choose.

Maybe in the future it’ll do even better in those areas, but for now it’s a fantastic way to organize and listen to your music and videos, and is without peer in that regard.   906. The new Zune browser is surprisingly good, but not as good as the iPod’s. It works well, but isn’t as fast as Safari, and has a clunkier interface. If you occasionally plan on using the web browser that’s not an issue, but if you’re planning to browse the web alot from your PMP then the iPod’s larger screen and better browser may be important.

  910. Apple now has Rhapsody as an app, which is a great start, but it is currently hampered by the inability to store locally on your iPod, and has a dismal 64kbps bit rate. If this changes, then it will somewhat negate this advantage for the Zune, but the 10 songs per month will still be a big plus in Zune Pass’ favor.
