Wszawica to choroba pasożytnicza wywoływana przez stawonogi, najczęściej wesz głowową. Ze względu na uporczywy świąd osoba zarażona często rozdrapuje skórę głowy, co może doprowadzić do wystąpienia zakażeń i stanów zapalnych.

Wszawica, inaczej pedikuloza, to choroba, która najczęściej dotyka dzieci i młodzieży. Bardzo łatwo się rozprzestrzenia. Zarazić się nią może każdy, niezależnie od tego, czy przestrzega zasad higieny. Pasożyty przenoszą się na innego żywiciela przez bezpośredni kontakt, m.in. zetknięcie się głów, lub pośrednio, np. przez kontakt z rzeczami osobistymi chorego, jego ubraniami, ręcznikami czy szczotkami. Rozprzestrzenianiu się wszawicy sprzyjają duże skupiska ludzi, dlatego do zarażeń najczęściej dochodzi w przedszkolach, szkołach, komunikacji miejskiej czy na koncertach.

Jak rozpoznać wszawicę?

Wszy głowowe mają około 3 mm wielkości i bardzo szybko się przemieszczają, dlatego trudno je zauważyć. Można jednak znaleźć przytwierdzone do nasady włosa jaja pasożyta, czyli gnidy.

Pierwszym objawem wszawicy jest uporczywy świąd, obejmujący zwłaszcza okolicę ciemieniową, skroniową i potyliczną głowy. Swędzenie jest skutkiem ukąszenia wszy, która podczas ssania krwi wprowadza pod skórę żywiciela toksyny. Skóra na linii włosów i za uszami może stać się zaczerwieniona.

Wskutek drapania spowodowanego świądem dochodzi do linijnych ubytków naskórka, czyli przeczosów. Rany często krwawią, sączą się i pokrywają strupami. W miejscu uszkodzeń może dojść do zakażeń bakteryjnych lub grzybiczych, rozwoju stanów zapalnych czy powstania liszaju. Infekcjom sprzyja zwłaszcza nieodpowiednia higiena. Jeżeli wystąpią miejscowe stany zapalne, chory może mieć powiększone węzły chłonne, m.in. za uszami.

Charakterystycznym objawem wszawicy jest również „kołtun” – włosy zlepione ropno-surowiczą wydzieliną.

Jak leczyć wszawicę?

W terapii wszawicy zaleca się stosowanie dostępnych w aptekach miejscowych preparatów antypasożytniczych, takich jak m.in. pyretryna czy permetryna. Można stosować środki w formie żelu, szamponu, kremu lub lotionu. Terapię należy powtórzyć po upływie tygodnia. Gnidy dojrzewają w ciągu siedmiu dni, dlatego istnieje ryzyko, że po tym czasie na włosach pojawią się nowe wszy.

Po każdorazowej aplikacji środka przeciwpasożytniczego należy wyczesać włosy gęstym grzebieniem w celu usunięcia martwych wszy i odklejonych gnid. Następnie należy pamiętać o zdezynfekowaniu grzebienia. Włosy można również płukać w roztworze octu i wody, by rozpuścić substancję scalającą gnidy z włosami.

Podczas leczenia stosuje się także preparaty zawierające oleje silikonowe, np. dimetikon. Środek ten pokrywa włosy warstwą nieprzepuszczającą powietrza, co skutkuje uduszeniem wszy i ich larw.

Do leczenia wszawicy nie można używać preparatów przeznaczonych dla zwierząt. Ich zastosowanie może podrażnić skórę głowy i wydłużyć czas gojenia ran.

Jeżeli w przebiegu choroby w rany wdało się zakażenie lub rozwinął się stan zapalny, należy zgłosić się do lekarza rodzinnego. Lekarz może zalecić stosowanie miejscowych środków przeciwzapalnych, a także preparatów przeciwświądowych, które pozwolą uniknąć wtórnych nadkażeń.

W jaki sposób zapobiec nawrotom wszawicy?

Po zakończeniu terapii należy zdezynfekować rzeczy i ubrania osoby zakażonej oraz jej najbliższych, takie jak środki do pielęgnacji, szczotki, nakrycia głowy, ozdoby do włosów, pościel czy pluszaki.

Odzież i pościel powinno się uprać w temperaturze co najmniej 60 stopni C. Warto zastosować przeciwpasożytnicze środki do prania oraz wyprasować ubrania żelazkiem z funkcją pary. Podczas prasowania należy zwrócić szczególną uwagę na obszary przy szwach.

Przedmioty, których nie można uprać, powinno się szczelnie zamknąć np. w plastykowej torebce i zostawić na około trzy tygodnie. Wszy pozbawione dostępu do żywiciela umrą. Powinno się także dokładnie odkurzyć miejsca, na które mogły przenieść się pasożyty, m.in. dywany, kanapy czy fotele.
