Właściwym stanem przednowotworowym skóry jest zarówno róg skórny, rogowacenie starcze skóry, jak również skóra pergaminowa. Jak powstaje i czym charakteryzuje się pierwszy z nich, czyli róg skórny?

Róg skórny jest twarda zmianą skórną, która powstaje z białka tworzącego górną warstwę skóry, zwanego keratyną. Swoim wyglądem może przypominać róg, stożek, guz czy kolec. Zazwyczaj zmiany te nie są zbyt duże, jednak mogą być zarówno małe, jak i duże, a także łagodne lub złośliwe. Kształtem przypominają rogi zwierzęcia.

Przyczyna jej powstania jest taka sama, jak przy rogowaceniu starczym, czyli narażenie na promieniowanie słoneczne, a także wirusowe brodawki wywołane wirusem brodawczaka ludzkiego. Zmiany te są odmianą rogowacenia słonecznego. Mogą powodować ból i świąd. Są charakterystycznego żółtawego lub brunatnego koloru. Ich kilkumilimetrowej podstawie towarzyszy zazwyczaj lekki naciek zapalny.

Róg skórny może również wystąpić u dzieci. Wówczas najczęstszą przyczyną wystąpienia jest stan zapalny lub długotrwałe drażnienie skóry. U tej grupy pacjentów nie zalicza się go natomiast do stanów przedrakowych.

Na podłożu tego rodzaju zmiany u 10-20 proc. seniorów dochodzi do rozwoju raka kolczystokomórkowego

Róg skórny może być również zmianą powstałą w wyniku nawarstwienia się mas rogowych, które tworzą różnego kształtu stożkowate twory. W przypadku potwierdzenia diagnozy przez specjalistę, zmiana powinna zostać usunięta chirurgicznie, laserowo lub za pomocą krioterapii, oraz poddana badaniu histopatologicznemu. Dobór metody usunięcia rogu skórnego będzie uzależniony od rozmiaru zmiany i jej charakteru.

Jeżeli badania wykazały stan przed lub nowotworowy, konieczny będzie zabieg chirurgiczny polegający na głębokim zamrażaniu lub laseroterapii. Ponadto leczenie nowotworowych rogów skóry obejmuje również chemioterapię, radioterapię oraz leczenie farmakologiczne.

Róg skórny może rozwinąć się na podłożu różnego rodzaju patologii skórnych

Rak skóry lub zmiany przedrakowe skóry są przyczyną połowy przypadków rogów skórnych. Zmiany te mogą również powstać na podłożu blizn, oparzeń bądź pozostałych nienowotworowych schorzeń skóry.
