Opracowane przez międzynarodowy zespół naukowców narzędzie SPOT to system punktacji histologicznej oparty na różnorodnych parametrach w każdej fazie gojenia. Dokładność i powtarzalność systemu została zweryfikowana w czterech różnych typach mysich modeli ran skóry (rany na podłożu cukrzycowym, rany związane z otyłością, odleżyny, rany oparzeniowe). Wyniki przeprowadzonych badań opublikowano w czasopiśmie naukowym „Stem Cells and Development”.
Założenia systemu SPOT
Nazwa systemu SPOT pochodzi od nazw czterech głównych ośrodków naukowych, których pracownicy brali udział w projekcie. Są to: Stellenbosch University (Republika Południowej Afryki), Polska Akademia Nauk, University of Texas Southwestern Medical Center (Stany Zjednoczone) oraz Obatala Sciences Inc. (Stany Zjednoczone).
W systemie uwzględniono następujące parametry gojenia ran:
– reepitelializacja,
– wskaźnik grubości nabłonka,
– keratynizacja,
– grubość ziarniny,
– stopień przebudowy tkanek,
– wskaźnik uniesienia blizny.
W ramach systemu badanemu przypadkowi można przyznać od 0 do 12 punktów, gdzie 0 to otwarta rana bez oznak gojenia, oceniana tuż po wystąpieniu urazu, natomiast 12 to całkowicie zamknięta i wygojona rana bez nadmiernego zbliznowacenia. Parametry mogą zostać ocenione za pomocą jednej z dwóch metod badania histopatologicznego: barwienia hematoksyliną i eozyną lub barwienia trójbarwnego wg Massona.
Pierwszy histologiczny system oceny ran
Jak zwracają uwagę autorzy projektu, pomimo dostępności wielu narzędzi do oceny klinicznej ran trudno gojących się, nie istniał do tej pory system oceny postępów gojenia ran oparty na parametrach histopatologicznych. Przeprowadzone badanie jest pierwszym, które statystycznie standaryzuje i waliduje tego rodzaju narzędzie. Autorzy podkreślają, że opracowany system punktacji został oparty na wskaźnikach rutynowo ocenianych w literaturze i dobranych w taki sposób, aby odpowiadały różnym fazom gojenia.
