Drakunkuloza to tropikalna, pasożytnicza choroba tkanki podskórnej, którą wywołują nitkowce Dracunculus medinensis. Schorzenie objawia się piekącymi i bolesnymi pęcherzami zlokalizowanymi na dolnych kończynach. Zmiany te mogą doprowadzić do zapalenia tkanki podskórnej, owrzodzenia czy wytworzenia ropnia i ropowicy.

Do zarażenia Dracunculus medinensis dochodzi poprzez spożycie zanieczyszczonej wody, w której znajdują się oczliki (skorupiaki słodkowodne) zainfekowane larwą nitkowca podskórnego. Po połknięciu przez człowieka larwy nicienia, pasożyty przedostają się do jelita, a następnie do naczyń limfatycznych i krwionośnych.

Jakie są objawy drakunkulozy?

Drakunkuloza w początkowym stadium jest bezobjawowa, jednak po około roku żeński osobnik pasożyta przedostaje się do skóry. Wówczas na skórze tworzą się bolesne, swędzące stwardnienia i pęcherze, które mogą ulegać owrzodzeniom. Pod skórą człowieka zazwyczaj pasożytuje od 1 do 4 dojrzałych samic o wielkości od 32 do nawet 120 cm długości i 0,5-2 mm średnicy. Zmianom skórnym mogą towarzyszyć objawy ogólne, takie jak stan zapalny węzłów chłonnych, gorączka, odczynowe zapalenie stawów, biegunka nudności i wymioty. Pęknięcie pęcherza może prowadzić do zakażenia rany, zapalenia tkanki podskórnej, a także do wytworzenia ropnia i ropowicy.

W trakcie przemieszczania się pasożyta do tkanki podskórnej mogą wystąpić reakcje alergiczne, takie jak pokrzywka, obrzęk, świąd, nudności, wymioty, biegunka, zawroty głowy, duszność, napady astmy, omdlenia, gorączka.

Jak przebiega leczenie podskórnej choroby pasożytniczej?

Drakunkuloza jest chorobą tropikalną, więc w postawieniu diagnozy niezwykle istotny jest wywiad lekarski. Narażone na chorobę mogą być osoby podróżujące do strefy endemicznej. Nicienie najczęściej spotykane są w Afryce. W praktyce klinicznej w diagnozowaniu drakunkulozy zlecane są badania mikroskopowe – wykrycie larw nicieni możliwe jest w płynie pochodzącym z pęcherza skórnego lub w tkance podskórnej.

W krajach rozwijających się nicienie usuwane są samodzielnie przez pacjentów poprzez nakręcanie pasożyta na zapałkę lub patyk. Proces ten może potrwać kilka tygodni. Natomiast w praktyce klinicznej nicienie usuwa się chirurgicznie. Zwykle nie zaleca się leków przeciwpasożytniczych, ponieważ wykazują one małą skuteczność w zwalczaniu Dracunculus medinensis. W przypadku objawów alergicznych zalecane są leki przeciwalergiczne.

Całkowite wyleczenie choroby następuje w momencie usunięcia pasożyta z organizmu. U niektórych pacjentów są jednak obserwowane powikłania, takie jak zbliznowacenia, zniekształcenia, ropnie, zwapnienia, owrzodzenia skórne i tkanki podskórnej.
