Rumień stwardniały to choroba, która objawia się w postaci przewlekłych skórnych zmian zapalnych. Występujące w przebiegu choroby guzy mogą ulegać wrzodzeniu. Jaka jest etiologia tego schorzenia i jak przebiega jego leczenie?

Przyczyny rumienia stwardniałego

W wielu przypadkach wystąpienie rumienia stwardniałego wiąże się z przebytą lub czynną gruźlicą. Uważa się, że produkty rozpadu prątków gruźliczych wywołują specyficzną reakcję układu immunologicznego, co ma wpływ na powstawanie wykwitów. Zmiany skórne o analogicznej charakterystyce morfologicznej mogą jednak występować również u pacjentów, u których nie stwierdzono gruźlicy. Przyczynę choroby stanowią w takim przypadku antygeny innych drobnoustrojów.

Choroba częściej występuje u kobiet niż u mężczyzn.

Objawy rumienia stwardniałego

Rumień stwardniały występuje pod postacią mnogich guzów zapalnych zlokalizowanych w tkance podskórnej. Skóra pokrywająca guzy ma sinoczerwoną barwę. Guzy są twarde i mogą wywoływać u pacjenta silny ból. Zmiany występują przede wszystkim w obrębie kończyn dolnych (głównie na podudziach po stronie zginaczy, rzadziej na udach) i mają charakter przewlekły. Choroba może ulegać okresowym zaostrzeniom.

Wyróżnia się dwie postacie rumienia stwardniałego – wrzodziejącą oraz niewrzodziejącą.

W pierwszym przypadku występujące w przebiegu choroby guzy ulegają rozpadowi i wrzodzeniu, których gojenie ma długotrwały i utrudniony przebieg. Owrzodzenia mogą ulegać nadkażeniom i goją się z pozostawieniem przebarwień i blizn.

W postaci niewrzodziejącej guzy ulegają samoistnemu wchłonięciu z pozostawieniem zagłębienia.

Leczenie rumienia stwardniałego

Leczenie rumienia stwardniałego ma charakter przyczynowy. Obejmuje przede wszystkim długotrwałe, wielolekowe leczenie farmakologiczne nakierowane na zwalczanie drobnoustrojów, które doprowadziły do rozwoju choroby. W przypadku gruźliczego podłoża rumienia wdraża się kilkumiesięczne leczenie przeciwprątkowe (izoniazyd, rifampicyna, pirazynamid).
